WHY TEST PRIVATELY

"Your thyroid is normal" usually means one number was normal

Standard NHS thyroid testing starts and usually stops with TSH. Most labs use reflex testing: free T4 is only added if the TSH comes back outside the reference range. Free T3 is rarely measured in primary care at all, and thyroid antibodies are ordered selectively rather than routinely.

For most people that is perfectly adequate. But if you have persistent thyroid symptoms and a TSH that sits inside the reference range, the standard test has told you very little — and there is no obvious next step, because the first result looked fine.

A full profile measures all four at once. That means you find out whether your antibodies are raised (the most common cause of an underactive thyroid in the UK is autoimmune, and antibodies can be positive years before TSH shifts), and whether your free T3 is low even when everything else looks unremarkable.

The four markers, and what each one tells you

TSH

The signal your pituitary sends to the thyroid. A raised TSH means your body is having to shout to get a response — often the earliest sign of an underactive thyroid, sometimes before you feel anything at all.

Free T4

The main hormone the thyroid actually produces, measured as the unbound portion your body can use. Read alongside TSH, it separates a thyroid that is struggling from one that is coping.

Free T3

The active hormone your cells use, converted from T4. Rarely measured in primary care — but it is the one that most directly reflects what is reaching your tissues.

TPO Antibodies

Identifies autoimmune thyroid disease, the most common cause of an underactive thyroid in the UK. Antibodies can be raised well before your other results change, which makes this the marker that explains the "why".

Common Questions

Because "normal" usually refers to TSH alone. A full profile adds free T4, free T3 and antibodies, which can show a picture the single reading doesn't — particularly raised antibodies, which often appear before TSH moves.

No fasting is required for a thyroid profile. Just come well hydrated. A morning appointment gives the most consistent TSH result if you plan to retest later.

Yes, and it's a common cause of misleading thyroid results. Biotin (often in hair, skin and nail supplements) can interfere with the assay. Stop it 48 hours before your appointment and tell us you've been taking it.

It can be, particularly if you still have symptoms on a dose your GP considers adequate — free T3 isn't usually monitored on the NHS. Take your levothyroxine after the blood draw rather than before, and keep your GP in the loop on any dose decisions.
