South America is where yellow fever advice has changed most in recent memory. Areas of Brazil once considered lower risk have seen the virus arrive, unvaccinated European travellers have been among the cases, and itineraries that mix cities, coast and rainforest need more careful reading than a single yes or no. Where the risk actually is

The WHO lists risk in areas of 13 countries across Central and South America: Argentina, Bolivia, Brazil, Colombia, Ecuador, French Guiana, Guyana, Panama, Paraguay, Peru, Suriname, Venezuela, and Trinidad and Tobago, the one Caribbean territory with risk. In almost all of them the star matters: risk sits in parts of the country, classically the Amazon basin and forested lowlands, while other regions carry low potential or none.

KEY POINT — altitude and geography do real work here. The same country can contain a no-risk capital, a low-risk coastline and a high-risk rainforest. “Do I need it for Peru?” has no honest one-word answer; “do I need it for this three-week route through Peru?” does.

What the recent outbreaks changed

From late 2017 Brazil experienced its most significant yellow fever activity in decades, with the virus reaching areas of São Paulo and other states previously considered lower risk. The WHO reported cases in unvaccinated European travellers, and further cases followed among visitors from Europe, Argentina and Chile. Earlier, returning travellers to Peru, Bolivia and Suriname, all unvaccinated, included fatalities.

Two lessons follow. Risk maps move, which is why recommendations are checked against live records rather than guidebooks, and the travellers who came to harm were consistently the unvaccinated ones. Certificates in South America: entry rules and onward travel

Brazil recommends vaccination for risk areas but does not currently demand a certificate from travellers arriving directly from the UK. The complication is onward and multi-country travel, which is how most South American trips are built. Cross from a risk country into a neighbour, and that neighbour may check; fly home via another continent and a transit rule can surface; and cruise itineraries apply each port’s rules to wherever the ship has recently been.

KEY POINT — think in sequences, not destinations. On a Lima–Cusco–Amazon–Rio itinerary the question is not “does Peru require it?” but “what does each border make of where I have just been?” We check every leg in order, against the current records, at your consultation.

Planning it from Bromley

The practical sequence is simple. Book your consultation when the itinerary firms up, ideally six weeks out. We map the route, decide with you whether yellow fever vaccination is recommended or required for any leg, and schedule it at least 10 days before the first border that could ask, remembering the certificate then lasts for life. Frequently asked questions

Do I need the yellow fever vaccine for Brazil?

For most Brazilian itineraries beyond the far south, vaccination is recommended, and recent outbreaks extended risk into areas once considered lower risk. Brazil does not currently demand the certificate from direct UK arrivals; your route is confirmed against the live country record.

Do I need it for Machu Picchu?

Not for the high Andes themselves. But most Peru trips that include the Amazon do need it for that leg, so the itinerary as a whole decides.

Is there yellow fever in Argentina or Chile?

Chile has none. Argentina has risk only in limited northern areas, including around Iguazu; Buenos Aires and Patagonia carry none.

What about the Caribbean legs of a South America cruise?

Trinidad is the one Caribbean territory with risk. Cruise rules apply port by port based on where the ship has been, so bring the full sailing itinerary.

I’m over 60 and visiting the Amazon. Can I be vaccinated?

Often yes, after careful assessment, because the Amazon is a genuine risk area where the vaccine can be justified at 60 plus. The full over-60 position is covered in our main guide and assessed individually at consultation.

How late can I book before a South America trip?

The certificate needs 10 days to become valid, so two weeks before departure is the practical minimum, and six weeks lets the whole vaccination plan breathe. Inside 10 days, contact us anyway and we will be honest about options.

Do children need it for South America?

Where the route carries risk, children are vaccinated from 9 months and receive their own lifelong certificate. Family scheduling should allow for the four-week gap between yellow fever and MMR.
